As you know I have been struggling with my health recently and am still unable to work. I have applied for ESA / PIP (or whatever) and think it would be useful if I had something ‘human’ from you explaining my condition.
I realise you have high workloads but wonder if you would produce a brief summary and a list of my various appointments and referrals. I would then be able to forward this to any interested parties.
I need to resolve this as I worry that the uncertainty and stress will have a negative effect on my health.
